2. Personal information and its handling

The personal information listed above is provided voluntarily and voluntarily.

4. Procedures for requests for disclosure of personal information

With regard to the personal information listed in 1. above that is held by our company, the individual or their agent may request "notification of the purpose of use, disclosure, correction, addition or deletion of the content, suspension of use, erasure, suspension of provision to third parties, and disclosure of records regarding provision to third parties."
Please contact the following office to receive requests for disclosure, etc. After receiving your request, we will inform you of the documents required for the procedure. Please note that you will need to submit the following identity verification documents to complete the procedure.
To verify your identity, please mail or bring in the following identity verification documents along with the "prescribed form" that we will provide you with separately.
